Millard West Activities and Athletics Online Forms

With first practices of the school year quickly approaching, it is important your son or daughter have the required paperwork turned into the office. This includesástudents who are planning on participating in athletics, lacrosse, cheer or dance.áThey are not allowed to participate without the required paperwork completed.áFirst step is to complete theáGoogle Forms link.áTo access the Millard West Activities and Athletics Online Forms, clickáHEREIncluded in the link is a Parent Permission and Agreement Form. This can be emailed toklchastain@mpsomaha.org.áThe Physical Clearance form and $60 participation fee must be turned into the Activities Office at Millard Westáas soon as possible. Do not wait until the first day of school. The office is openáMondayáthrougháFriday7:00-3:00. Any paperwork will NOT be accepted at the open house night.á

If your son or daughter is participating in Show Choir, Debate or Forensics, only the $60 participation fee is required. Please bring this to the Activities Office as soon as possible.á
